Coaching Agreement / Informed Consent

By enrolling in a coaching session, program, or consultation with Health Factors, you acknowledge and agree that:

  1. Coaching is not medical care and does not replace medical diagnosis, treatment, or prescriptions.

  2. You are responsible for consulting your physician before starting any nutrition, supplement, or lifestyle program.

  3. You understand that results depend on individual effort and adherence.

  4. Confidentiality is maintained; your data will not be shared without your consent.

  5. Health Factors and its coaches are not liable for any direct or indirect outcomes arising from coaching sessions.

By continuing, you provide informed consent and voluntarily participate in the program.
